2026-2027 Parent Input Form

In determining classroom placement, we will consider your insights, our observations at school and the need for classroom balance. Completion of this form is optional. We will give individual attention to every student's placement, whether this form is completed or not. Parents/guardians wishing to share input about their child's placement should return fill this form and submit NO LATER than 11:59PM May 10, 2026. Please do not use teacher names in your responses with the exception of question 4. Requests for specific teachers will not be considered.
